InterMed teamed up with Aetna Insurance for a project that involved 1,500 patients over the course of three years. Staff members at InterMed set aside time each week to go through patient records, look ahead to see what preventative care they need to avoid a medical crisis, and then form a relationship with the patient to encourage them to seek help before they end up in the hospital.
The InterMed staff works closely with Aetna to make sure preventative claims are covered. InterMed says they have a great working relationship with staff at Aetna, and they've seen hospital stays and emergency room visits drop 50-percent since this program started.
InterMed says they've seen an average annual savings of more than $600 per patient with 1,500 patients taking part in the 3-year program that's a total savings of close to a million dollars.
